Objectives: To describe the diurnal and nocturnal findings of 4 adults (3 women) and one child with systemic lupus erythematosus and shrinking lung syndrome. Design: A prospective observational study. Setting: A 750 teaching-bed-hospital with a reference population of 350,000 people. Interventions: Clinical history, blood analysis, imaging test, pulmonary function tests, maximal inspiratory and expiratory pressures, arterial blood gases, and sleep studies. Hematologic manifestation of systemic lupus erythematosus
Systemic lupus erythematosus (SLE) is a multisystem autoimmune disease. The most common hematologic finding is anemia, leukopenia, thrombocytopenia and secondary antiphospholipid syndrome with recurrent abortion and thrombosis. The autoimmune fibrosis of bone marrow is another manifestation of autoimmune disease especially SLE, that must be correctly differentiated from primary myelofibrosis.

Shrinking lung syndrome (SLS) is a rare manifestation of systemic lupus erythematosus. We report the case of a patient with non-responding SLS (neither to glucocorticoids nor immunosupresors), who showed remarkable improvement after the onset of treatment with rituximab. Although there is a little evidence, treatment with rituximab could be proposed in SLS when classical treatment fails.
